How do I do the payments?

All consultations are bulk-billed for patients presenting with a valid Medicare card.

Some procedures will have a small cost that you will be informed about before you are booked in.

When can I speak to the doctor?

You can contact the Doctors at the Centre during normal operating hours. The Receptionist will take a message if the Doctor is attended with another patient and in cases of emergency your call will be put through to the Doctor.

How do I get a medical certificate?

Medical certificates are legal documents and cannot be issued without actually seeing the Doctor. These certificates cannot be issued retrospectively.

Who can access my medical records?

All medical records are strictly confidential and the contents cannot be disclosed to anyone without your consent or where justified by law. Southlakes Medical Practice complies with the Privacy Act 1988 including the ways we collect, store, use and disclose health information.

How can I give feedback?

If you have any suggestions to improve the Practice, you are invited to speak to the Doctor directly. We believe that problems are best dealt with in the Practice.

Can I see the doctor if I do not have a Medicare card?

Appointments can be made for patients that do not have a Medicare care. There is a small fee which you can ask about when you book your appointment.

How do I make the payments?

Patients without a medicare card will be charged a small fee. Please ask the receptionist for cost

How can I access care outside practice opening hours?

When our practice is closed, we recommend that you access the After-Hours GP Clinic located at the Day Surgery Unit of the Dubbo Base Hospital. It is open on weekends and most public holidays from 2.00pm to 5.45pm. The local GP Phone Service operates from 6.00pm to 8.00am on weekdays and 24 hours on weekends and public holidays (Ph: 02 6884 2100). You can also phone Health Direct 24/7 (1800 022 222).

Can I complete my new patient registration form before my first appointment

-Yes you can download a copy of the form from the website then scan and e mail to admin@southlakesmedical.com.au ( Please do not use paint or take a photo to send ) or bring the paper copy with you to your appointment.
